Players under 18

Every player under 18 needs three things in addition to the normal details:

  • The parent/guardian consent form, signed.
  • A letter from the LC1 Chairperson of the village.
  • The guardian's and the LC1 Chairperson's names and phone numbers.

How it works

Four steps from sign-up to match day.

    Register

    Fill in the online form or upload your filled PDF. Add a clear photo and, if you are under 18, the signed consent and LC1 letter.

  2. 2

    Get your reference

    You receive a number like PR-2026-000123. Print or screenshot the acknowledgement.

  3. 3

    Review

    The Sports Office checks your details and documents. Track it on the status page with your reference and the last 4 digits of your phone.

  4. 4

    Play

    Approved players can be added to their team's squad by the team manager, and officials get an accreditation card.

Questions

Do I need a National ID number (NIN)?

Yes, from age 18 - 14 characters starting with CM or CF. Under-18s who do not have one yet may leave it blank.

Can I play more than one sport?

Yes. Tick every sport you want to take part in; you can be in the squad of a different team in each sport if your managers agree.

My village is not in the list.

Choose "Other (not in the list)" and type it. The Sports Office will match it to the correct village later.

I do not have a team yet.

Choose "Not yet in a team". Team managers can only add you to their squad once your registration is approved and your team is selected, so ask your manager to tell you the exact team name.

How is my information protected?

We collect only what is needed to run the championship, under the Uganda Data Protection and Privacy Act, 2019. Consent forms and LC1 letters are stored privately and are never shown publicly.

The upload says my form is invalid.

Only unmodified forms downloaded from this site work, and each form can be used once. Download a fresh copy or fill the online form instead.
