We, the undersigned, on behalf of the above-named team, hereby acknowledge and declare that:
- We have received, read and understood the Mukono Municipality Mayor's Cup 2026 Tournament Rules and Regulations in their entirety.
- We agree to abide by all rules, regulations and decisions of the Organising Committee, Technical Committee, Disciplinary Committee and Appeals Panel.
- We accept that the decisions of the Appeals Panel are final and binding.
- We undertake to ensure that all players, coaches, team officials and supporters associated with our team conduct themselves in accordance with these regulations and the spirit of fair play.
- We understand that violations of these regulations may result in sanctions including fines, suspensions, forfeits and expulsion from the tournament.
- We accept collective responsibility for the conduct of our supporters and pledge to appoint and support at least two Fan Marshals.
- We consent to the use of our players' and officials' images, names and likenesses in official tournament communications and promotions.
- We acknowledge the inherent risks associated with sporting activities and agree to the waiver of liability provisions in Section 15.9 of the regulations.
